Survey 7

The University of Sydney would like to thank you for taking part in the CAP Study surveys at school over the past 6 years. Your participation in the study has been very important in helping us to better understand and reduce harms among young people.

You are invited to complete the last CAP Study survey. You will be reimbursed $30 upon completion of the survey, and go in the draw to win an iPhone each year. This survey asks you questions about yourself and about your activities, and your reasons for engaging in those activities. The purpose of the survey is to understand different experiences such as the use of alcohol and other drugs and how they interact over the long term. If you decide to participate, all you will need to do is complete this confidential questionnaire. It should take no longer than 40-60 minutes, and will ask general questions about your personality, behaviour, and the activities you engage in.

All information you provide will be strictly confidential, except as required by law and/or duty of care.

Once again, thank you for your previous and ongoing participation.

The CAP team.
